Immunotherapy ‘Altering the Landscape’ for Bladder Cancer

More evidence that immunotherapy with programmed cell death (PD) inhibitors is effective in bladder cancer was presented here today at the 2016 European Society for Medical Oncology (ESMO) Congress.

The results come from two new phase 2 trials. The CheckMate-275 study showed nivolumab (Opdivo, Bristol-Myers Squibb) was active in patients with metastatic bladder cancer who progressed despite first-line platinum-based chemotherapy, while the KEYNOTE-052 study showed pembrolizumab (Keytruda, Merck) was active as first-line therapy in cisplatin-ineligible patients with metastatic or locally advanced bladder cancer.

The companies are expected to use data from these trials to obtain approval for bladder cancer as a new indication for the agents, which are already approved for melanoma and lung cancer.

The first immunotherapy for bladder cancer, atezolizumab (Tecentriq, Genentech), was approved by the US Food and Drug Administration earlier this year. The decision was based on the IMvigor 210 trial for use in patients with locally advanced or metastatic disease that has progressed during or following platinum-containing chemotherapy or within 12 months of receiving neoadjuvant or adjuvant platinum-containing chemotherapy.

It seems that every company is conducting a phase 2 study with its own immunotherapy to get a bladder cancer indication, commented session cochair Bernard Escudier, MD, from the Institut Gustave Roussy, Villejuif, France, but “we need phase 3 trials,” he told Medscape Medical News in an interview.

“Everything that we have seen in the past few years shows that immunotherapy is active in bladder cancer — probably more active than chemotherapy, at least overall,” he said. All the evidence suggests that immunotherapy is important in bladder cancer, he added, “but it is still important to know which patients will benefit more from one than the other, because we have some patients who benefit a lot from chemotherapy.”

In the first instance, he feels a randomized clinical trial of standard chemotherapy vs immunotherapy as first-line treatment in bladder cancer is needed — and is being planned. The next step would be to look at combinations of these therapies.

So far, the PD-L1 biomarker has not proven to be very useful in selecting patients who may respond to immunotherapy, he said, as patients who are PD-L1-negative sometimes respond. There is some suggestion that, for patients who are PD-L1-negative, it would be best to start with a combination of therapies, while in those who are PD-L1-positive, it may be reasonable to begin with immunotherapy as monotherapy, Dr Escudier commented. But he agreed with some of the other speakers that the question of which drugs to use is mainly a clinical decision. For example, in a patient with a high burden of disease, it is probably better to start with chemotherapy to try to shrink the tumor and then add immunotherapy. “This is what we do,” Dr Escudier noted.



Israeli-German Discovery Could Help Cure Deadly Skin Cancer

A new discovery by Israeli and German researchers could be the key to stop the spread of melanoma, the deadliest kind of skin cancer.
The scientists found the method by which melanoma spreads from its original site and how to stop the metastasis before it turns deadly, according to a study published online Monday in Nature Cell Biology.
“The threat of melanoma is not in the initial tumor that appears on the skin, but rather in its metastasis — in the tumor cells sent off to colonize in vital organs like the brain, lungs, liver and bones,” said the study’s lead researcher, Dr. Carmit Levy, according to The Jerusalem Post.
“We have discovered how the cancer spreads to distant organs and found ways to stop the process before the metastatic stage,” added Levy, of Tel Aviv University’s Sackler School of Medicine.
The Tel Aviv University researchers collaborated closely with scientists at two Israeli hospitals — the Sheba Medical Center in Ramat Gan and the Wolfson Medical Center in Holon — as well as the German Cancer Research Center in Heidelberg.
Prior to metastasizing, melanoma tumors send out vesicles containing small RNA particles that cause changes in the surrounding skin, according to the findings.
Melanoma kills one person every 52 minutes, according to the Skin Cancer Foundation. Despite ongoing research, there is still no cure for the deadly disease.
Levy said researchers have known for years that melanoma forms in the epidermis, the outer layer of the skin, but the new research sheds light on how the process occurs in the dermis, the skin’s inner layer.
The changes prepare the inner layer of skin surrounding the original tumor to receive and transport the tumor cells, the scientists discovered.
The Israeli research team found two chemicals that can stop the cancer from spreading, which they say could be used in future medical treatments.
“Our study is an important step on the road to a full remedy for the deadliest skin cancer,” Levy said. “We hope that our findings will help turn melanoma into a nonthreatening, easily curable disease.”

Genes to Predict Breast Cancer Survival

The activity of two genes may help predict certain breast cancer patients’ chances of survival and guide their treatment.

Researchers looked at tumors from nearly 2,000 breast cancer patients, focusing on her-2 positive breast cancers. This type accounts for about 20 percent of all breast cancers.

People whose tumors had high activity in a gene called F-12, but low activity in a gene called S-T-C-2 were three times more likely to die within 10 years compared to patients with different patterns of activity in these two genes.

The team concluded specific patterns of gene activity may play an important role in enabling breast cancer cells to spread throughout the body.

They hope the findings may be used to develop tests for aggressive breast cancers or identify new targets for treatment.

New class of anti-cancer agents may be promising treatment for multiple myeloma

Australian researchers have discovered that a new class of anti-cancer agents may be effective in treating multiple myeloma, an incurable bone marrow cancer.

The research revealed that the majority of myelomas rely on a protein called MCL-1 to stay alive. Potential drugs that inhibit MCL-1, which are in pre-clinical development, may be a promising new treatment for multiple myeloma.

Each year more than 1700 Australians are diagnosed with multiple myeloma, which is a cancer of immune cells called plasma cells. Currently available treatments are only able to halt the progression of the disease and relieve symptoms, but cannot cure the disease.

Walter and Eliza Hall Institute researchers Dr Jianan Gong, Dr David Segal, Ms Yuan Yao, Professor Andrew Roberts and Professor David Huang, working with researchers at the Australian Centre for Blood Diseases, Monash University and the Alfred Hospital, investigated the ‘survival proteins’ that keep myeloma cells alive, allowing the cancer to persist.

The survival of many cancer types relies on a family of related survival proteins called BCL-2 family proteins, Professor Huang said. “In the past decade there has been considerable interest in the using anti-cancer agents called ‘BH3-mimetics’ to kill cancer cells by blocking the BCL-2 family proteins,” he said. “Recent clinical trials have demonstrated that a BH3-mimetic that switches off the protein BCL-2 is an effective treatment for certain forms of leukaemia.

“Our latest research has focused on which BH3-mimetics would be the most effective in treating multiple myeloma, a cancer for which new treatments are urgently needed,” Professor Huang said.

Dr Gong said the team discovered that the majority of myeloma samples died when MCL-1 was switched off. “In contrast, only around one-quarter were susceptible to inhibiting BCL-2,” she said. “This finding is in keeping with earlier research at the Walter and Eliza Hall Institute that pinpointed MCL-1 as the likely protein that keeps myeloma cells alive.

“Our research shows that switching off MCL-1 has the potential to be effective new treatment approach for the majority of patients with myeloma,” Dr Gong said.

Professor Roberts said that there had been considerable interest in developing anti-cancer agents that target MCL-1. “As yet, these inhibitors are still in pre-clinical development,” he said. “Our results suggest that, once necessary laboratory testing for safety is completed, clinical trials of their effectiveness in treating patients with multiple myeloma that is no longer responding to current therapies would be well justified.”

Vitamin B17 Controversy : Poison or Cancer Treatment?

Banned by the FDA in the 1980s but touted by some alternative medicine practitioners as a treatment for cancer, the controversy over the innocently named vitamin B17 rages on. Once labeled and marketed as a vitamin, its appearance practically disappeared without a trace within the mainstream medical community.

Today, however, a simple Web search pulls multiple blogs and articles either generously supporting this nutrient as a miracle cure or vilifying it as a hoax.

Vitamin B17, also called amygdalin or laetrile, is a glycoside nutrient linked with cancerprevention in alternative medicine practices — and there are anecdotal claims that it’s actually cured cancer. Vitamin B17 is derived from natural food sources and most abundant in seeds of plants of the prunasin family, such as apricots and apples.

Vitamin B17 interacts with other antioxidants — including vitamin A, vitamin C andvitamin E — along with pancreatic enzymes to break down and eliminate harmful cells from the body. This makes it beneficial for detox support, immunity and potentially even various forms of disease prevention.

Vitamin B17, which has the scientific name mandelonitrile beta-D-gentiobioside, is considered a nitriloside, a natural cyanide-containing substance. Laetrile, the extract form of vitamin B17, is most well-known for potentially helping prevent cancer development through the production of hydrogen cyanide.

This beneficial compound is released into the body’s tissues and targets and destroys mutated cells. However, patients weren’t followed long term to determine whether or not the benefits continued after treatment stopped, so it’s hard to tell whether vitamin B17 could act as a natural pain reliever for other conditions, such as arthritis. (4)

4. Lowers High Blood Pressure

Vitamin B17 may cause a low blood pressure reaction due to formation of thiocyanate, a powerful blood pressure–lowering agent. However, it’s unknown if this is an effective treatment long-term or if the effects are mostly temporary.

Once metabolized, vitamin B17 causes enzyme beta-glucosidase production that interacts with intestinal bacteria to detox the body and lower blood pressure. This normally isn’t a danger for most people and may be beneficial for some, but it’s important not to use vitamin B17 in this way if you already take blood pressure-lowering medication.

If you have any existing heart issues that could become complicated if you experienced a sudden drop in blood pressure, you should avoid taking vitamin B17.

Is Vitamin B17 Treatment New?

Vitamin B17 is far from new. “Bitter almonds,” another source of vitamin B17, have been used as a traditional remedy for thousands of years by cultures, including ancient Egyptians, Chinese and Pueblo Indians. The compounds in vitamin B17 were discovered around 1802 when a chemist realized that distilling the water from bitter almonds released hydrocyanic acid and this could be purified to form amygdalin, the active ingredient of vitamin B17.

Vitamin B17 in the form of laetrile was first used as a cancer treatment in Russia back in the mid-1800s and then spread to the United States in the 1920s. By the 1970s, laetrile gained popularity as an anti-cancer agent, with more than 70,000 individuals in the U.S. alone using vitamin B17 laetrile to help treat cancer.

Today, vitamin B17 laetrile is not approved for cancer prevention orcancer treatment use in the United States. That’s because there’s little evidence to fully understand how laetrile works in humans and prove that it’s definitely safe and effective.

While vitamin B17 shows anti-cancer activity in some animal studies, the FDA feels that more information is needed regarding the effects of vitamin B17 in human clinical trials before it can be widely used to prevent disease and increase immunity.

While it’s a banned substance for retailers to sell, it’s not illegal to possess or use. Therefore, some practitioners still use vitamin B17 in the form of laetrile to help treat cancer. They often obtain these supplements and extracts from Mexico, where vitamin B17 production for medicinal purposed is still supported.

Recommended Intake of Vitamin B17

At this time, there is no established daily value of vitamin B17 from the USDA. However, many physicians specializing in cancer treatment use vitamin B17 (or laetrile) in relatively high doses without patients commonly experiencing side effects.

Vitamin B17, in the form of amygdalin, given intravenously at up to 4.5 grams a day produced no clinical or laboratory evidence of toxic reactions, according to one report published in the Journal of the American Medical Association. (8) Other studies show similar results and only report cases of toxicity when very high doses were given, causing cyanide poisoning.

Types of Vitamin B17 Supplements

Vitamin B17, or laetrile extract, can be administered orally as a pill, or it can be given by injections (intravenous or intramuscular). Most often vitamin B17 is given intravenously for a short period of time and then followed up with lower doses of oral tablets for maintenance therapy.

In the medical community, vitamin B17 injections are usually used to help prevent ortreat cancer, although these are extremely expensive, costing thousands of dollars for only a few months of treatment. A popular way to consume vitamin B17 is eating apricot kernels. In the middle of an apricot, or other fruit seeds like a peach pit or apple seed, there is a hard shell that can be broken into. Once broken, a small seed/kernel in the middle is found that looks something like a small almond — this is the part of the fruit that’s naturally high in vitamin B17.

Some people choose to purchase high quantities of apricot kernels over the Internet or pills and liquid supplements made from apricot kernels. Experts usually recommend eating 25–40 kernels per day for disease prevention or about 16 kernels and up for maintenance.

Cyanide is a neurotoxin that causes a range of side effects, including nausea and vomiting, headaches, dizziness, discoloration of the skin resulting from oxygen-deprived hemoglobin in the blood, liver damage, abnormally low blood pressure, mental confusion, and even death.

Oral vitamin B17 is considered more dangerous and prone to cyanide poising than injected laetrile. These side effects are increased by eating raw almonds or crushed fruit pits, or by eating fruits and vegetables that contain beta-glucosidase enzymes — including celery, peaches, bean sprouts and carrots.

High doses of vitamin C can also add to harmful side effects when taking vitamin B17. On the other hand, consuming foods that contain acid, specifically hydrochloric acid, helps prevent side effects of vitamin B17. These include citrus fruits like lemon, orange or grapefruit.

A couple of serious warnings to keep in mind regarding interactions of vitamin B17 include the fact that it can lower blood pressure drastically in some cases and also cause blood thinning. So it should never be used with other blood pressure medications or prescriptions known to thin blood. It’s also not recommended to take vitamin B17 withprobiotics because probiotics may enhance the effects of cyanide and lead to cyanide poisoning in some rare cases.

Any night work tended to boost the adjusted odds of breast cancer 40 percent compared with never working nights, reported Johnni Hansen, PhD, and Christina F. Lassen, MD, both of the Danish Cancer Society in Copenhagen.

But that risk rose with more years of night shift work and a higher cumulative number of night shifts worked, more than doubling in risk for women who had the night shift most weeknights for at least 6 years.

The risk was particularly great for “morning people” who had to work intense night shifts, they reported online in Occupational and Environmental Medicine.

Night work seems to be on the rise worldwide, with 10 percent to 20 percent of the workforce in Western countries now working schedules that start after 5 p.m. and end before 9 a.m., the group noted.

Most, though not all, prior case-control and large prospective studies have also suggested a risk for breast cancer from such shift work.

“Night work can disrupt circadian rhythms, suppress production of the pineal hormone melatonin and result in sleep deprivation, all of which affect hundreds of metabolic and physiological processes, including hormone production, cell cycling and apoptosis, which in turn may increase the initiation, progression and growth of human tumors, including breast cancer,” they wrote.

As part of a larger study of occupational and non-occupational exposures and risks for cancer in the Danish military, Lassen’s group compared 218 cases of breast cancer to 899 matched controls within a nationwide cohort of 18,551 female military employees.

Night shift work, defined as lasting for at least 1 year but not including overtime, didn’t have a significant impact if women worked it only once or twice a week.

“This is consistent with the observation that one or two night shifts will not change the timing of melatonin production and thereby not initiate circadian disruption,” Hansen and Lassen noted.

But longer duration — night shift work at least three times a week — boosted risk.

The odds were 2.1 times higher for 6 to less than 15 years on that schedule and 2.5 times greater with 15 or more years of at least three nights a week, both statistically significant adjusted risks compared with never being on the night shift.

The risk rose with greater cumulative exposure too.

Self-selection bias has been suggested as a possible explanation for these associations if “night owls” are more likely to accept or stay on the night shift and are more genetically susceptible to breast cancer than “larks”, women who prefer mornings.

However, the study showed greater risk actually among the larks than among the night owls.

Inadequate vitamin D from missing out on sun exposure by working nights has also been proposed as a mechanism.

Again, though, the study did not favor that explanation, finding that the night workers were more likely to report sunbathing.

Further exploration is warranted in larger studies, the researchers argued.

They cautioned that this retrospective study may have suffered from potential recall bias. Another limitation was the modest rate of participation of military members in the questionnaire the study was based on.

Also, women who had died from breast cancer were not included, which may have underestimated the associations.

Don’t rely on routine tests alone to protect you from cancer. It’s just as important to listen to your body and notice anything that’s different, odd, or unexplainable.

1. Wheezing or shortness of breath
One of the first signs many lung cancer patients remember noticing is the inability to catch their breath.

2. Chronic cough or chest pain
Several types of cancer, including leukemia and lung tumors, can cause symptoms that mimic a bad cough or bronchitis. Some lung cancer patients report chest pain that extends up into the shoulder or down the arm.

3. Frequent fevers or infections
These can be signs of leukemia, a cancer of the blood cells that starts in the bone marrow. Leukemia causes the marrow to produce abnormal white blood cells, sapping your body’s infection-fighting capabilities.

4. Difficulty swallowing
Trouble swallowing is most commonly associated with esophageal or throat cancer, and is sometimes one of the first signs of lung cancer, too.

5. Swollen lymph nodes or lumps on the neck, underarm, or groin
Enlarged lymph nodes indicate changes in the lymphatic system, which can be a sign of cancer.

6. Excessive bruising or bleeding that doesn’t stop
This symptom usually suggests something abnormal happening with the platelets and red blood cells, which can be a sign of leukemia. Over time, leukemia cells crowd out red blood cells and platelets, impairing your blood’s ability to carry oxygen and clot.

7. Weakness and fatigue
Generalized fatigue and weakness is a symptom of so many different kinds of cancer that you’ll need to look at it in combination with other symptoms. But any time you feel exhausted without explanation and it doesn’t respond to getting more sleep, talk to your doctor.

8. Bloating or abdominal weight gain
Women diagnosed with ovarian cancer overwhelmingly report unexplained abdominal bloating that came on fairly suddenly and continued on and off over a long period of time.

9. Feeling full and unable to eat
This is another tip-off to ovarian cancer; women say they have no appetite and can’t eat, even when they haven’t eaten for some time.

10. Pelvic or abdominal pain
Pain and cramping in the pelvis and abdomen can go hand in hand with the bloating that often signals ovarian cancer. Leukemia can also cause abdominal pain resulting from an enlarged spleen.

11. Rectal bleeding or blood in stool
This is a common result of diagnosing colorectal cancer. Blood in the toilet alone is reason to call your doctor and schedule a colonoscopy.

12. Unexplained weight loss
Weight loss is an early sign of colon and other digestive cancers; it’s also a sign of cancer that’s spread to the liver, affecting your appetite and the ability of your body to rid itself of wastes.

13. Upset stomach or stomachache
Stomach cramps or frequent upset stomachs may indicate colorectal cancer.

14. A red, sore, or swollen breast
These symptoms can indicate inflammatory breast cancer. Call your doctor about any unexplained changes to your breasts.

15. Nipple changes
One of the most common changes women remember noticing before being diagnosed with breast cancer is a nipple that began to appear flattened, inverted, or turned sideways.

16. Unusually heavy or painful periods or bleeding between periods
Many women report this as the tip-off to endometrial or uterine cancer. Ask for a transvaginal ultrasound if you suspect something more than routine heavy periods.

17. Swelling of facial features
Some patients with lung cancer report noticing puffiness, swelling, or redness in the face. Small cell lung tumors commonly block blood vessels in the chest, preventing blood from flowing freely from your head and face.

18. A sore or skin lump that doesn’t heal, becomes crusty, or bleeds easily
Familiarize yourself with the different types of skin cancer — melanoma, basal cell carcinoma, and squamous cell carcinoma — and be vigilant about checking skin all over your body for odd-looking growths or spots.

19. Changes in nails
Unexplained changes to the fingernails can be a sign of several types of cancer. A brown or black streak or dot under the nail can indicate skin cancer, while newly discovered “clubbing”– enlargement of the ends of the fingers with nails that curve down over the tips — can be a sign of lung cancer. Pale or white nails can sometimes be a sign of liver cancer.

20. Pain in the back or lower right side
Many cancer patients say this was the first sign of liver cancer. Breast cancer is also often diagnosed via back pain, which can occur when a breast tumor presses backward into the chest, or when the cancer spreads to the spine or ribs.

Whether you are a man or a woman, it’s important to watch for any unusual changes in your body and energy levels in order to detect any signs of cancer early on. The sooner you notice there’s a problem, the sooner you can begin to take the steps necessary to promote healing within your body.

Of course, ideally you should follow an anti-cancer lifestyle even before you notice any symptoms, as prevention is the best route when it comes to most chronic diseases. It is not unusual for 10 or more years to pass between exposure to a cancer-causing agent (tobacco, chemicals, radiation, cell phones, poor nutrition, etc.) and detectable cancer.

So during this time you have a chance to alter the progression of the disease.

Cancer is actually a group of diseases characterized by uncontrolled growth and spread of abnormal cells. The “cure” lies in controlling this abnormal growth and stopping the spread.

Your body has a remarkable capacity to do just that — to heal — and that ability is fueled largely by your lifestyle. If you eat well, exercise, get enough sleep and sun exposure and address your emotional stress, your body should be able to maintain a healthy balance.

The problem with cancer often lies not only with ignoring these health principles but also with the invasive and highly risky treatments that conventional medicine relies on to treat it — surgery, chemotherapy and radiation.

This may surprise you to hear, but a recent landmark study found some cancers, even invasive cancers, may go away without treatment, and it may happen more often than anyone thought.

On the contrary, many experts now say cancer patients are more likely to die from cancer treatments like chemotherapy than the cancer itself.

The alarming rates of cancer deaths across the world — cancer has a mortality rate of 90 percent, according to Italian oncologist Dr. Tullio Simoncini — speak volumes about the effectiveness, or lack thereof, of these treatments, yet they are still regarded as the gold standard of cancer care.
Cancer Rates Continue to Rise in 2009
Nearly 1.5 million new cancer cases are expected to be diagnosed in the United States in 2009, not including the over 1 million cases of basal and squamous cell skin cancers that are also expected to be diagnosed this year, according to American Cancer Society data.

In all, more than 1,500 people will die from cancer each day, accounting for nearly one out of every four U.S. deaths.

The latest estimates show that by 2030, over 26 million people a year may be diagnosed with cancer, with 17 million people dying from it.

Many of these cancer cases and deaths can be prevented, however.

Even the American Cancer Society states that about one-third of the more than 562,000 cancer deaths expected to occur in 2009 will be related to overweight or obesity, physical inactivity and poor nutrition. Another 169,000 will be caused by tobacco use.

There is another widely overlooked cause of cancer deaths, one that could easily be changed as well, and that is a lack of vitamin D from sun exposure.

We now know that well in excess of half of cancer cases would simply disappear if vitamin D levels were optimized.

As I said earlier, living an anti-cancer lifestyle now will help your body to stay healthy and in balance, and avoid developing cancer in the first place. Here are the top tips I recommend to start living your healthier lifestyle now:
1. Normalize your vitamin D levels by getting plenty of sunlight exposure and consider careful supplementation when this is not possible. If you take oral vitamin D and have a cancer, it would be very prudent to monitor your vitamin D blood levels regularly.

2. Reduce or eliminate your processed food, sugar and grain carbohydrate intake. Yes, this is even true for whole unprocessed organic grains, as they tend to rapidly break down and drive your insulin and leptin levels up, which is the last thing you need to have happening if you are seeking to resolve or prevent cancer.

3. Control your fasting insulin and leptin levels. This is the end result, and can be easily monitored with the use of simple and relatively inexpensive blood tests.

4. Normalize your ratio of omega-3 to omega-6 fats by taking a high-quality animal-based omega-3 fat like krill oil and reducing your intake of most processed vegetable oils.
